  • Referrals to the best specialists when needed. We refer to UCSF, Stanford, Sutter, and John Muir depending on your situation.
  • Access medication refills, annual exam reminders, and lab/imaging requisitions and results through our Patient Portal.
  • Direct pay is required at the time of service
  • We will file claims for your reimbursement with commercial insurance as an “out of network provider”
  • You will receive an Explanation of Benefits (EOB) from your insurance company
  • Being out-of-network may result in higher out-of-pocket costs
  • Medicare-covered services will be billed directly to Medicare
  • You are responsible for copays, coinsurance, and deductibles as determined by Medicare
  • We will submit claims to Medicare for all office services
  • Medigap or supplemental insurance may help cover some costs not fully paid by Medicare

Onboarding Process

  • We will contact you to discuss the services and expectations of Private Medicine Lafayette for both the physician and member (onboarding discussion) —Free, no charge, schedule at Patient Portal.
  • Onboarding documents include consent to treat, privacy agreement, HIPAA agreement, and financial agreement for your review and signature. These documents are kept at your portal for your reference.
  • If a service agreement is reached after the initial onboarding discussion, your medical record will be requested and reviewed. Diagnostic laboratory, a colon cancer screen, and a cardiac calcium scan will be ordered. An initial comprehensive office visit/executive physical will then be scheduled.
  • The concierge/membership fee ($2250) will be due on the day of the comprehensive 90-minute exam (refundable on a prorated basis unconditionally at any time). Schedule at Patient Portal.
  • During the onboarding and data collection period—typically two-four weeks—members are invited to communicate openly and explore health goals with Dr. Hood. The membership fee is collected only after the comprehensive initial exam, allowing both patient and physician to build trust and determine if the practice is a good mutual fit. If, during this initial period, medical needs arise that require more frequent prescriptions or additional services before the comprehensive exam, Dr. Hood may discuss collecting the annual fee earlier. In rare circumstances, if care expectations or needs seem outside the scope of the membership, Dr. Hood may recommend considering whether the membership is the right fit for both parties.

Benefits and Facilities

  • A complimentary facility tour and review of services will be provided during the onboarding process.
  • Following onboarding, laboratory results, imaging studies including a cardiac calcium scan, and EKG findings will be discussed in detail using a shared large screen for clarity and ease of understanding.
  • A comprehensive medical record will be established, which includes your complete medical history, allergies, current medications, vaccinations, review of systems, problem list, assessment/diagnosis, and treatment plan.
  • Dr. Hood has developed a proprietary, HIPAA-compliant EMR that adheres to CMS guidelines.
  • An after-visit summary (AVS) will be generated after every office or telemedicine visit.
  • An active medication, drug allergy, and vaccination list is maintained within the EMR to ensure comprehensive care.
  • Electronic prescriptions are sent directly to your pharmacy of choice, typically for a 90-day supply with five refills when indicated, minimizing the need for frequent office visits for refills.

  • Wellness in Private Medicine Lafayette means living better through fitness and athletic performance. This is an added benefit to the Concierge subscription medical service at a nominal rate.
  • Private Medicine Lafayette has a MicroFit FAS-2 Fitness Testing Lab which uses research and protocols from the American College of Sports Medicine (ACSM) and Stanford University and is an FDA registered device.
  • Increase athletic performance including endurance, flexibility, strength, balance, body composition, and lowering body mass.
  • Wellness and fitness testing is great for men and women of any age seeking to improve their athletic performance.
  • Complimentary medical kit for home telemedicine vital signs and for travel emergencies. Includes blood pressure cuff, 02 saturation monitor, thermometer…cold/URI meds, pain and fever meds, diarrhea, and constipation medications.
  • Evaluation of international travel plan with recommended vaccines and prophylactic medicines.
  • Monday – Saturday telemedicine available when you cannot come to the office.
    • Disclaimer – Telemedicine, although convenient and popular, is not comprehensive and is suboptimal to an office examination.
  • Dr. Hood strongly prefers to see patients in person at the office whenever possible. This allows for a more comprehensive and personal approach to your healthcare. However, there are specific situations when telemedicine is the better option. If you’re actively infectious with conditions such as a cold, the flu, or COVID-19, we kindly ask that you stay home and utilize our telemedicine services. This approach helps prevent the spread of contagious illnesses and protects other patients and our staff while ensuring you still receive the care you need.

  • Fully equipped procedure room for minor surgical interventions
  • Removal or biopsy or skin cancers with necessary margins including basal cell, squamous cell, and melanoma with dermatopathology at UCSF
  • Removal of various skin growths, including cysts, lipomas, skin tags, and lesions
  • Drainage of boils and abscesses
  • Suture and treatment of lacerations and wounds
  • Fees determined based on the complexity of each procedure

If you do not file an insurance claim, you will be eligible for our direct pay rate, which is typically lower and fully transparent and determined before the procedure.

If you choose to file an out-of-network claim, the charges submitted to insurance will be higher, consistent with prevailing community standards for such services. Your out-of-pocket costs may also be higher, depending on your plan.

Cosmetic procedures, such as removal of cysts and skin tags, are not covered by commercial insurance or Medicare and must be paid for directly by the patient.
