Private Medicine Lafayette is onboarding twenty-five new patients per month beginning June 2022.
The practice will be limited to no more than 300 members (national average Concierge profile is 500-1000 patients).
Fees
$2050 a year for membership, refundable on prorated basis at any time if you are not satisfied.

$100 for wellness fitness testing visit for members.
$400 for non-member wellness fitness testing visit.
Minor office surgery cost per complexity of procedure.
Onboarding
We will call you to discuss the services and expectations of Private Medicine Lafayette for both the physician and member (onboarding discussion) —Free, no charge, schedule at (Patient Portal)
If a service agreement is reached after the initial onboarding discussion, an initial comprehensive office visit/executive physical will be scheduled for a $100 deposit.
The concierge/membership fee ($2050) will be due on the date of service. The $2050/year concierge fee is refundable on a prorated basis unconditionallyat any time during the year from the initial office visit for the following twelve months. Schedule at (Patient Portal)
